Auto Accident & Worker's Comp Accepted!

We accept Auto Accident claims and Worker's Comp claims at all locations.

When booking your appointment please have the following information available:

1. Your name, address and contact information

2. Claim Number

3. Insurance Company Name and Address

4. Insurance Adjusters Name and Contact number

5. Date of Accident

6. If you have retained an attorney, we will need his/her Name & contact number

7. Patients will be charged $30 for any missed or rescheduled appointment if less than 24 hrs notice is given.

8. You will need to bring in your prescription for service either on the day of service or fax to our office at 302-994-2916.
